Quality Assurance Office

Three distinct ways to question a standard, report a standards concern or document interference with work.

Public working journey · 17 October 2026

Not merely a place to send complaints. A place to keep a question alive until it can be answered responsibly, corrected, or explicitly marked unresolved.

Discuss a standard

Ask about a source, scope, interpretation, applicability, conflicting evidence or a correction to the public library.

What the Office can and cannot decide

The Quality Assurance Office is a proposed organizational standards and build-integrity function. It is not a government authority, tribunal, regulator or certification body; it cannot decide a criminal allegation, employment claim, copyright ownership, disability diagnosis or legal finding merely by receiving a report. An independent incident-report service remains distinct from standards conversations.

Published source material is readable without submitting personal information. Do not place private reports, witness identities, medical information or proprietary source code in a public correction request.

The accountable five-stage journey

  1. Identify the expectation: source, edition, exact claim or promised behavior.
  2. Compare against evidence: what was directly observed, what was inferred, relevant records and contrary explanations.
  3. Establish responsibility: who can investigate or correct, with what authority and limits.
  4. Record decisions and corrections: preserve the original and date each material change; distinguish proposal from finding.
  5. Verify the result: test the actual experience and keep unresolved limitations visible.

This is the intended governance journey; operational case handling and turnaround are not claimed as verified simply because this page is published. A report should remain open until a documented outcome or explicit unresolved status exists in the authorized system.

Safety and independence

There is no requirement to seek agreement from an accused person or to engage in a debate before reporting. Equally, a report does not establish wrongdoing. Genuine emergencies, filing deadlines, formal notices, legal rights and independent authorities are not displaced. Email channels have different delivery and receipt behavior from the numbered incident form.
